Release of Information/ Medical Records Forms: Please fill out the correct form. If requesting medical records from us, bring, mail or fax the form to us. If requesting medical records from someone else to be sent to us, please take, mail or fax the form to the other party (or give it to us to send to the other party).

Patient Portal: We have a free Patient Portal for you! It makes it possible for you to view your medical records. You can also ask us questions securely online or send us a secure e-mail. We can respond securely. Ask us for an invitation if you have not yet joined. It is available to any patient we have seen. The portal is housed within FollowMyHealth, which is used by many physicians. With one login, you may be able to link to many of your physicians. It is the easiest and fastest way to communicate with us. You can email us any time of day or night, and we will respond quickly, usually the same day and definitely within one working day. No more waiting on hold or transfer to voice-mail!

Fees: Our fees are much lower than fees charged by hospital centers in the area. We are very transparent about our fees. We disclose to you our fees, what your insurance company is likely to approve and pay, and what you are likely to have to pay, in advance of any testing. Please keep in mind that the information we provide you is based on what your insurance company tells us, and the information they provide may be inaccurate or change without notice. You are ultimately responsible for payments of your deductibles, co-pays, co-insurances, and non-covered procedures. Rest assured that the amount you will have to pay us is usually less than the amount you will have to pay at a hospital center. It is almost never more than the amount you will have to pay anywhere else.
